We are excited that your student will be joining us for Student Life Summer Camp in Glorieta, NM from July 9 - 14, 2018! Here is everything you need to know to get your student ready for camp:
Each student will need to fill out three forms - one for Student Life that needs to be printed and notarized... and one for Glorieta Conference Center that can be completed online... and the medical release form that will be kept by Avenue Student Ministry Staff. Please see the links below to fill them out.
If your child will take medication at camp, a medication form will need to be completed as well. If you need a medication form for your student, you can request it by emailing email@example.com. We will also have those forms available upon check-in.
Each student will need to bring $40 cash - $20 will be used for meals on the way to New Mexico and the other $20 will be turned into Avenue Student Ministry Staff upon check in. That $20 will be handed back to each student when we load the buses to return home. It will pay for their meals on the way back.
Plan to deliver your student to The Avenue Church on Monday, July 9 at 10:30pm. Our buses will leave the parking lot at 1:30am. We have rented charter buses and have professional drivers who will be transporting us. Each bus will have two drivers in accordance with bus safety regulations. We will leave Glorieta at 8:30am on Saturday to return. Our estimated arrival is 10:00pm on Saturday, July 14.

FINAL BALANCE and all PAPERWORK is DUE NO LATER than June 20, 2018.
In the event that you are unable to print the attached forms, we will have all of them in a camp packet available on the porch during our weekend experiences.
We have a notary available in the church office (210 YMCA Dr) during the week, Monday - Thursday from 8:30am-5:00pm.
